Duik rig, head keeps getting detached from body

Hello I am trying to rig this dude and the head keeps getting detached from the body.
It happened in the past sometimes, not sure why it's happening

What makes it move like that exactly? What were you doing when the head got detached?

Hi, I'm just moving the head controller (it's selected in the picture)
Is it meant only for rotation?

I just realized that when I don't make a neck armature it's working fine
Any ideas why the neck is messing the rig?
